Proof of immunization requirements

The Maine Department of Education requires “all children enrolled in a public or private school” to have certain immunizations, depending on the child’s grade level. See the Maine DOE’s list of required immunizations for details.

Proof of immunization must include the name of the child, date of birth, and must be “from a physician, nurse practitioner or physician assistant or public health official who has administered the immunizing agent(s) to the child. The certificate must specify the immunizing agent, the dosage administered and the date(s) on which it was administered.”

You can either attach the proof of immunization to the health form as part of the enrollment process, or have your health care provider send the proof via fax directly to the district registrar's FAX number: 207-781-1012

Exemptions or special circumstances

There are exemptions available for medical purposes, per MSRS 20-A(2).

Religious or philosophical exemptions are only allowed if they were in place prior to September 1, 2021 and part of a student’s IEP, according to MRS 20-A section 6355 (3 & 4) and the Maine Department of Education

Resources for parents

Portland Public Health Division Clinic offers free immunizations to all adults and children. Vaccinations are by appointment, Monday-Friday, 8:30 AM to 5:30 PM.
