The following forms are the new State of Maine required special education forms.

• Written Notice (this form is used as a notice to parents for several different reasons, one of which is Consent for Initial Placement)

• Parental Consent for Evaluation (this consent form grants the school permission to evaluate a child)

• New Individualized Educational Program (this is the new IEP developed by the State of Maine)

• Referral to Special Education Form
